Basketball Camp

Get ready for kids basketball!
We will be holding a mini basketball camp open to boys and girls in 7th, 8th, and 9th grade. Saturday Nov 9, 2024 from 9AM to 4PM

Danny Burd, Founder and Ministry Director of Mpact Sports will be conducting the camp.
The cost is $20 for first child and $15 for each additional child.

I hereby consent that my child, who is under 18 years of age, may participate in Mpact Sports Ministry BASKETBALL CAMP. In addition to this consent, on behalf of my heirs, executives, and assigns, I hereby release Lifeline Church, Mpact Sports Ministry basketball camp, its shareholders, executives, trustees, agents, employees, and assigns from any and all liability that may arise by virtue of this consent. I acknowledge and agree that all requirements, directions, supervisions, and standards set by the directors of this program shall be established for my child’s benefit. Additionally, I hereby assume all risk of accident or injury to my child, which may arise out of his or her participation in the program, and agree to indemnify New Promise Church, Mpact Sports Ministry, its shareholders, executives, trustees, agents, employees, and assigns for any liability Mpact Sports Ministry incurs as a result of my child’s participation in the Mpact Sports Ministry Basketball Camp. Furthermore, if I cannot be reached, I give Lifeline Church and Mpact Sports Ministry staff permission to act according to their best judgment in any emergency situation and authorize any and all medical procedures recommended by the health care professional to be necessary to sustain the health and safety of my child.
